To find a Dental Network Provider, select the network your plan currently uses. If you are unsure of your network, please refer to the back of your Dental identification card.

You can get an interpreter at no cost to talk to your doctor or health plan. To get an interpreter contact the Plan at 1.800.451.2513. Someone who speaks your language can help you.
